J. Thomas Davis of Georgetown D.C. having claim to the service or labor of the undersigned, and having refused or failed to file in your office the statement or schedule provided for in the 9th section of the act of Congress, entitled "An act for the release of certain persons held to service or labor in the District of Columbia" Approved 16th April 1862.
You will please file the annexed schedule in pursuance of an act supplementary to the above recited act approved 12 July 1862. The undersigned resided in the District of Columbia before and at the time of the passage of said supplemental act with said owners consent.

Henry C. Thomas, summoned. I know the servant Mary, daughter of my father's servt. I believe she belonged to J Thomas Davis of Georgetown DC. I do not know whether the girl was in District of Col. 16 Apl.
Lucy Berry (cold) This is my child. belonged to Mr Davis at time of Emancipation. lived with me never out of District. Mr Davis knew she was with me. sent for her last fall and I took her home.
