Mr George Burns having claim to the Service or labor of the undersigned and having refused or failed to file in your office the Statement or schedule provided for in the 9th Section of the act of congress entiled to an act for the release of certain persons held to service or labor in the District of Columbia Approved April 16 1862.
you will file the annexed Schedule in pursuance of an act Supplementary to the above recited act approved 12th July 1862. the undersigned resided in said District before and at the time of the passage of the Said Supplemental act with said owners consent.
