Sir,

Dr. Septimus J. Cook of Prince Georges Co. Md. having claim to the service or labor of the undersigned, and having refused or failed to file in your office the statement or schedule provided for in the 9th section of the act of Congress, entitled "an Act for the release of certain persons held to service or labor in the District of Columbia" approved 16th April 1862.

You will please file the annexed schedule in pursuance of an act supplementary to the above recited act approved 12th July 1862. The undersigned resided in said District before and at the time of the passage of the said supplemental act with said owner's consent.

Mrs. John (Sarah) Stiles I know servant belonged to Dr. Cook came to live with me 22 Feby. was here at time of passage of Emancipation Act. child was with her 3 years old was before clerk the other day

Mary says she had permission to hire but Master took it away in [illegible] carried her away & kept me until August—Mr Cook lives in Beltsville—has not tried to get me since. When he took my papers [carried?] me.

Mr Sam A Philips produces papers showing [illegible] 22 May 1862 that she had [illegible] middle of Jany 1862 witness says 1 Feby 62. servt​ was [illegible] 9 June
